What is a Geriatric Care Manager?

A professional geriatric care manager  (PGCM) is a health and human services professional. She is a guide, an advocate and resource for families caring for older relatives. The PGCM assists older adults and persons with disabilities to reach their maximum functional potential. An Individual’s independence is maintained to a degree determined by safety. The PGCM is knowledgeable about costs, quality and availability of resources in the community.

Many times, an individual‘s support system is compromised because the caregiver is overburdened or lives at a distance. The caregiver turns to a PGCM for assistance.
